Scope

The Specialist Allied Health and Behaviour Support Provider Scheme is seeking suppliers to gain pre-qualification to provide the following services:

· Exercise physiology

· Occupational Therapy

· Physiotherapy

· Speech Pathology

· Behaviour Practitioner

Professional association accreditations * (required)

Are all practitioners in your organisation registered with relevant professional association accreditations?  Therapists must meet the following requirements relevant to their profession:

• exercise physiologists are to be accredited with Exercise and Sports Science Australia (ESSA)
• physiotherapists, psychologists, and occupational therapists are to be unconditionally registered with the Australian Health Practitioner Regulation Agency (AHPRA)
• speech pathologists are to be members of Speech Pathology Australia.

• a description of the work undertaken and the outcomes and impact achieved
• the client organisation name, contact name, position, and contact phone or email
• contract duration, including the start & end date of the work.

The Evaluation Team may contact the clients as referees to assist in evaluating responses.
